Physicians Who Dispense Schedule II Drugs

Effective July 1, 2004, for each prescription for a Schedule II controlled substance that is dispensed by a physician, the physician shall record and maintain the following information:

  1. Full name, address, gender, and date of birth of the patient.
  2. The physician’s category of licensure and license number; federal controlled substance registration number; and the state medical license number of any physician using the federal controlled substance registration number of a government-exempt facility.
  3. Pharmacy prescription number, license number, and federal controlled substance registration number.
  4. NDC (National Drug Code) number of the controlled substance dispensed.
  5. Quantity of the controlled substance dispensed.
  6. ICD-9 (diagnosis code), if available.
  7. Date of dispensing of the prescription.

Effective July 1, 2004, each physician who dispenses Schedule II controlled substances must also provide the Department of Justice this information on a monthly basis in either hard copy or electronic form.
